Blood Pressure/Blood Sugar Clinics

Sponsored by Highpoint Health Home Health & Hospice, free blood pressure and blood sugar clinics are available to the general public at the following times and locations:  

First Tuesday of the month
9:30 a.m. | Hoosier Boy Apartments Community Room (Rising Sun)
11:00 a.m. | Dillsboro Village Apartments

Second Tuesday of the month
10:00 a.m. | Zion Lutheran Manor (Lawrenceburg)

Second Friday of the month

Third Thursday of the month
10:15 a.m. | North Dearborn Village Apartments (SNAC)

Fourth Tuesday of the month
10:00 a.m. | Switzerland County Senior Center (SNAC)

Participants should take their medications as scheduled.  Fasting for eight hours prior to the blood sugar screening provides more accurate results.  People with diabetes or those who are under a health professional’s care should consult their physician/nurse practitioner before fasting.  All participants are given their screening results which they are encouraged to share with their healthcare provider.

Breast-Feeding Class

A course designed to teach expectant parents the importance of breastfeeding and proper breastfeeding techniques.  The class is taught by International Board Certified Lactation Consultants.

Cardiac Rehabilitation (Phase II) Classes

A series of classes with coordinated instruction and exercise for patients who are recovering from a heart attack, bypass surgery or other related cardiac incident or procedure. Patients with a cardiac diagnosis, such as stable angina, are also appropriate for the class.

Childbirth Education

A basic course of the physiological, physical and emotional changes in a woman's body during pregnancy.  This course will also provide information on exercise and nutrition as well as preparing for childbirth.  Pre-registration is required.

Diabetes Education Classes

For many people, living with diabetes can be overwhelming and challenging, but it doesn’t have to be. Highpoint Health's Certified Diabetes Educators are here to help you on your journey as you manage your diabetes.

The following classes are taught by Certified Diabetes Educators and are recognized by the American Diabetes Association as meeting the national standards for diabetes self-management education. A physician’s referral is required and there is a fee to attend.  Most insurance companies will pay for diabetes classes with a physician’s order.  Please verify coverage with your insurance company ahead of time.

We encourage you to bring an adult support person (at no additional charge).

Living with Diabetes

Living with Diabetes is a comprehensive small group diabetes self-management class for adults with newly diagnosed diabetes or adults with a previous diagnosis of diabetes who have little diabetes knowledge and have not attended a class previously. The class is four 2-hour sessions. Topics include: What is Diabetes? Healthy Eating, Being Active, Medications, Monitoring, Dealing with Stress/Emotions, Preventing Problems, Reducing Risks and Changing Behavior.  The class is taught by a Registered Nurse and Dietitian who are Certified Diabetes Educators. Registration is required and there is a fee. A written physician Diabetes Self-Management Training referral is required for billing purposes. Evening and daytime classes are available. Another adult support person may attend at no additional charge. Participant or support person should be able to read and do basic math (label reading).

Diabetes Management Review

Diabetes Management Review is a two-hour small group class for adults with a previous diagnosis of diabetes who have some diabetes knowledge and/or have attended a class previously. The class is taught by a Registered Nurse and Registered Dietitian who are Certified Diabetes Educators. Registration is required and there is a fee. A written physician Diabetes Self-Management Training referral is required for billing purposes. Another support person may attend at no additional charge. Participant or support should be able to read and do basic math.

Gestational Diabetes Education

A one or two-hour session taught by a Registered Dietitian and a Registered Nurse, if monitoring and medication are needed. The Dietitian or Nurse will call the patient to schedule. There is a fee for this class. A written physician Diabetes Self-Management Training referral is required. Another adult person may attend at no additional charge.

Individual Diabetes Education

Available for those who cannot attend a group class due to a physician documented cognitive or physical issue (physical debilitation, mental illness, cognitive impairment, illiteracy, etc), for insulin instruction or highly individualized nutrition education (advanced carb counting).  Registration is required and there is a fee. A written Diabetes Self-Management Training physician referral is required.

Procedure for Diabetes Self-Management Training (DSMT)

  1. A written provider order/referral is required. Fax the referral to 812-537-009 or 812-537-8444.  The diabetes educators will call the patient to schedule the classes.
  2. The provider order/referral must contain all the required elements as CMS and private insurance has specific requirements for reimbursement (ICD10 code, A1C, Ht, Wt, and diabetes treatment plan). Medicare will cover small group classes only, unless there is a special circumstance that prohibits a group setting (see page 2). The Highpoint Health DSMT referral form contains all the required information. It is available on the DCH Intranet under “Nutrition Services”.  An Allscripts referral must contain all these elements.

 

Moving Toward Control

Moving Toward Control is a support group for people with diabetes or pre-diabetes. The group meets quarterly from 5:30 to 7:00 pm.

Pulmonary Rehabilitation (Phase II) Classes

A series of classes combining physical reconditioning and self-management education for patients diagnosed with chronic lung disease. Patients with a pulmonary diagnosis such as asthma, chronic bronchitis, emphysema, pulmonary fibrosis, and sarcoidosis are appropriate for the class.  Participation requires a physician's order.
